What is SysmConnect Online Consultation?

SystmConnect is an online service through which Parkside Medical Centre patients can contact the surgery to get help for Non-Urgent medical problems. 

You can expect a triaged response on the same day of the submission. 

Those patients requesting an URGENT clinical appointment or review should continue to contact us in person or via the telephone.

No. You can submit a request without creating an account or logging in.

The form allows you to choose between 2 options, ‘Medical Request’ or ‘Admin Request’.

Within the ‘Admin’ choice, you will find:

  • Request a doctor’s letter
  • Request a fit (sick) note
  • Follow-up on Test Results (blood test, scans, etc.)
  • Any other advice or information that is not medically related e.g. discuss referrals

Within the ‘Medical’ choice, you will find:

  • New conditions
  • Medicine queries

Once you have selected an option, you will be asked further questions about your request. The forms are only available during the practice working hours i.e. 8am - 6.30pm

If you need urgent treatment, you should call 999, visit A&E or attend an urgent treatment centre (UTC).

999 and A&E are for life-threatening emergencies like:

  • Loss of consciousness
  • Chest pain
  • Breathing problems
  • Severe bleeding
  • Severe allergic reactions
  • Severe burns or scalds
  • Strokes

Urgent treatment centres can help with things like:

  • Sprains and strains
  • Suspected broken bones
  • Injuries, cuts and bruises
  • Stomach pain, vomiting and diarrhoea
  • Skin infections and rashes
  • High temperature in children and adults
  • Mental health concerns

Please let us know when submitting the form that you require a translator - this is one of the questions on the form.

Our reception phone lines are very busy, SystmConnect offers another way in which you can contact the practice. 

Medical queries will be submitted to our triage team. A member of the team will respond to you the same day with with out outcome, this may be in the form of a booking link for an appointment or a response giving specific instructions about the next steps. Conditions that need to be assessed on the same day will be prioritised. The triage team may send you a request for further information in the form of a short questionnaire specific to your issue or you may be sent you a Self Booking Link, this is enable you to book a future appointment. 

Please only submit one medical issue per contact form, so that you can add sufficient detail for each problem, to allow our team to appropriately deal with your request. 

There is an option to submit an admin query on the online form for general administrative issues such as requesting a letter from a doctor and fit notes. A member of the Patient Support Team will respond to you within 5 working days.
